DOT Physical Exam Process

DOT physicals at our Bakersfield clinic are streamlined so that you can avoid unnecessary delays in receiving your DOT medical card. All of our providers are DOT-certified through the Federal Motor Carrier Safety Association, so you can rest assured that your exam will be “legit and official.”

  • A Department of Transportation (DOT) exam is a medical evaluation required by the Federal Motor Carrier Safety Association (FMSCA) to verify that one is healthy enough to safely drive a commercial motor vehicle (CMV).

    At our Bakersfield office we have a simple 3-step process to keep your appointment as hassle-free as possible. The entire DOT exam lasts less than 30 minutes from start to finish.

    DOT Physical Steps:

    #1: Complete the Medical Examination Report Form. This single page form is the only paperwork you’ll need to complete. You’ll be asked to supply your contact information, medical history and any medications that are currently prescribed to you. 

    (If you’re in a hurry you can print and complete the form prior to your appointment.) 

    #2: Next, our DOT-certified provider will perform a basic hands-on exam. This will consist of the following:

    >Vital signs (blood pressure and heart rate)
    >Vision (distance, peripheral and color recognition)
    >Hearing
    >Heart
    >Lungs (breathing)
    >Back/Spine (range-of-motion)
    >Legs/Arms (mobility, strength and reflexes)
    >Neurological 
    >Overall well being
    >Urinalysis (this is not a drug test)

    #3: Upon successful completion of the history and exam, we complete your paperwork. Sign all the necessary documents including your new DOT medical card. Your information will be uploaded into the National Registry.

  • In Bakersfield (and all of California) a DOT medical card can be valid for up to a maximum of two year. Some medical conditions do restrict a medical card to less time. The most common reasons for a reduced medical card are hypertension, diabetes, obstructive sleep apnea and heart disease.

    It’s imperative to renew a DOT medical card before it expires or one risks hefty fines and/or getting a vehicle “pulled off the road.” Bakersfield CDL holders also risk getting their commercial license downgraded to a Class D driver’s license with a lapsed medical card.

  • To avoid any delay in receiving a DOT medical card, you'll want to bring a few things to your appointment, including (if applicable):

    >Driver's license (or other form of photo ID)
    >Glasses and/or contacts
    >Hearing aids
    >A full bladder (for the basic urinalysis - not a drug test)
    >Medication list
    >Most recent Ha1C (if a diabetic)
    >Form MCSA-5870 if an insulin-dependent diabetic
    >CPAP compliance log (if diagnosed with sleep apnea)
    >Additional paperwork/medical clearances may be needed if you have a history of heart disease, stroke, seizures or other neurological disorders).

  • If you hold a commercial driver’s license in Bakersfield, CA, you are also required to submit a copy of your valid DOT medical card to the California DMV. This process is commonly referred to as self-certification and can be completed online or via mail every two years.

    CDL holders can self-certify by completing form DL-694 and either mailing it the address below or dropping it off at a DMV branch location.

  • Call us today to schedule your exam. Appointments are required but we can typically accommodate any same-day or next-day needs. Upon successful completion of the physical, you will receive your DOT medical card at the end of your appointment.
